HOW INSURANCE IS HANDLED
- If you have a deductible, payment is expected in full at the time of services.
- As a courtesy, I will send claims
to the insurance company for reimbursement of my services and/or to
indicate payment towards your deductible.
- If you have secondary insurance coverage, payment is expected in full at the time of service. I will send claims to the insurance company and you will be reimbursed directly from your insurance company.

Benefits Confirmation Sheet link below. Print out the sheet, call your
insurance company for the
information to fill in the sheet, and
bring the sheet with
you to your first visit.
